Overview
Older adults benefit from physiotherapy differently than younger patients. Strength and balance decline gradually and often unnoticed until a fall, an illness, or a hospital stay reveals how much has been lost. Our geriatric care focuses on prevention as much as treatment — helping older adults keep the independence they have, and rebuilding what has been lost when illness has knocked things sideways.
We work with patients over 60 for conditions including osteoarthritis, osteoporosis, post-fracture recovery, post-stroke care, Parkinson's, general deconditioning, and fall prevention. Many of our patients live with adult children in Vellore for a few months at a time and want a plan that will continue when they return to their home town — we write plans that travel well.
Every session is paced. We watch for fatigue, blood pressure changes, joint tolerance, and cognitive load. A safe, sustainable dose that happens four times a week beats an ambitious plan that happens once and hurts for three days.

We think about medication interactions carefully in older patients. Blood pressure medications, blood thinners, and diabetic medications all interact with exercise physiology. We ask for a medication list at the first visit and adjust intensity accordingly.
For older adults who live alone or with a spouse who also has mobility challenges, we can involve a paid caregiver in sessions so they know how to support daily practice safely.
Cognitive changes matter too. If a patient has memory difficulties, we use photo-based exercise sheets, keep instructions short, and involve family in every session so home practice actually happens as prescribed.
Medication awareness: blood pressure medications, blood thinners, and diabetic medications all interact with exercise physiology. We ask for a current medication list at the first visit and adjust intensity, sequencing, and monitoring accordingly.
Comorbidity-aware planning: cardiac disease, diabetes, and osteoporosis are common in this age group. We coordinate with the treating cardiologist, endocrinologist, or physician when the exercise plan needs adjustment or when a symptom change needs review.
Cognitive support: if a patient has memory difficulties, we use photo-based exercise sheets, keep instructions short, and involve family in every session. Home practice only works if it is realistic for the household.
Outcome tracking: Timed Up and Go, hand-grip strength, walking speed, and self-reported fall history are documented at intake and every four weeks. Objective tracking helps families and referring doctors see progress that day-to-day observation can miss.
Evidence base in brief: structured balance and strength training (for example, the Otago Exercise Programme) reduces fall rates by 30 to 40 percent in randomised trials when done consistently for twelve weeks in adults over 65. Our balance programs are built on this evidence.
Fall history review: at intake we ask about every fall in the last twelve months — location, time of day, footwear, and consequences. Fall pattern often reveals the biggest opportunity for prevention.
Red-flag surveillance: sudden falls with loss of consciousness, new incontinence, or sudden weakness are escalated to a physician the same day. Not every mobility problem is a physio problem.
Family involvement: we include one family member or paid caregiver in the last ten minutes of every session so home practice is safe, consistent, and reduces fall risk between visits.
Home environment review: at the first home visit we walk through the house looking at lighting, floor surfaces, bathroom set-up, and bedroom access. Small changes — a bedside night-light, a non-slip bath mat, a grab bar near the toilet — are often the single biggest fall-prevention intervention.
Bone health coordination: for patients with osteoporosis we coordinate with the treating physician regarding calcium, vitamin D, and DEXA-scan intervals. Exercise plans for low-bone-density patients avoid loaded spinal flexion and include weight-bearing and resistance training that is safe for the fracture risk profile.
Community re-integration: getting back to the temple, the market, or a daily walk to the neighbour's house is often the goal that matters most to older patients. We plan sessions toward specific community destinations rather than generic strength targets.
