What is Aquatic & Hydrotherapy?

Aquatic physiotherapy uses the properties of water — buoyancy, hydrostatic pressure, warmth and viscosity — to make rehabilitation possible when land-based exercise is too painful, too high-impact or too unstable. Buoyancy unloads joints. Hydrostatic pressure reduces swelling. Warmth relaxes muscles. Viscosity gives graded resistance in every direction.

At Dr. Karolin Rockson PT we run supervised aquatic-therapy sessions in a partnered warm-water pool in Vellore for patients who benefit most — knee and hip osteoarthritis, chronic low back pain, fibromyalgia, ankylosing spondylitis, post-fracture and post-operative rehab, and neurological conditions like incomplete spinal-cord injury, MS and stroke.

Every aquatic session is preceded by a land-based assessment to plan goals and ensure safety (no open wounds, no unmanaged incontinence, no uncontrolled seizures, no uncontrolled cardiac symptoms). Sessions run 30–45 minutes with a therapist in the water.

Water supports body weight so joints can move through range that would be too painful on land — ideal for early post-op and severe OA.

Progressive resistance

What to expect on day one

  1. Step 1. Land-based assessment first — screening for safety contraindications and setting pool-session goals.
  2. Step 2. Pool-side orientation, water-entry and safety brief.
  3. Step 3. Trial aquatic session with a therapist in the water — usually 20–30 minutes for first-timers.
  4. Step 4. Post-session debrief and land-side home programme.
  5. Step 5. Session-cadence plan — typically 1–2 pool sessions per week alongside land physio.

Water properties we use therapeutically: buoyancy (unloads 50–90% of body weight depending on depth), hydrostatic pressure (reduces peripheral oedema, improves venous return, supports posture), viscosity (graded resistance in every direction), and warmth (32–34°C reduces muscle tone and improves comfort).

Halliwick method: a systematic approach originally developed for children with disabilities, now used across neurological rehab. Teaches mental adjustment, balance control, movement in water and independence — all through structured play.

Bad Ragaz Ring Method: uses floats to support the patient in supine while the therapist provides proximal control and resistance. Effective for spinal conditions, post-op rehab and neurological cases.

Ai Chi: aquatic version of Tai Chi, useful for chronic pain and elderly balance. Slow flowing movements coordinated with breathing.

Post-op aquatic protocols: after TKR, aquatic walking can typically begin at 2–4 weeks (after wound healing and drain removal); after THR similar; after ACL, aquatic sessions typically start at 4–6 weeks with restricted movements per surgical protocol.

Safety and infection control: partnered pools maintain WHO-recommended chlorine and pH levels; patients with open wounds or active infections do not enter. Waterproof dressings are not sufficient for aquatic-therapy participation.

Bridging to land: aquatic gains transfer to land best when land-based sessions run alongside. We typically prescribe 1–2 aquatic + 1 land session per week, converging to land-only as function improves.

Evidence base: aquatic therapy has Level I evidence for knee OA pain and function (Cochrane 2016), Level I for hip OA, Level II for fibromyalgia, Level II for chronic low back pain, and Level II for post-TKR early rehab.
